The Department of Public Safety collaborates with the Physical Plant to maintain a safe and secure campus environment. We encourage faculty, staff, and students to fill out the applicable forms listed below.

Staff

For standard keys or electronic access, please complete the form and route for signatures.  After signatures are acquired the form should be emailed to security@lcsc.edu.  The key/access recipient will be notified when the keys are ready to be picked up.

The Vehicle Use Agreement is required for any individual driving an LCSC owned, rented, or leased vehicle. 

Please download and save the Vehicle Use Agreement before completing.  Complete the form, digitally sign and click the green approve button to automatically route to the next email listed (Supervisor).  The form will route to Public Safety when approved by the Supervisor (coordinator/dept. head/director).  VP/Pres. signature only required if the driver is an off-campus volunteer.  A current copy of the driver's license is required.  Directions are attached to the pdf document on how to create a digital signature and how to securely upload a driver's license copy via LeepFile.  Please use clear color copies only.  Check the box for Van Training if you will be driving a 15-Passenger van.  You will be set up to view a 21-minute training video in Vector Solutions.
